These Terms and Conditions govern your participation in treatments, workshops, classes, and sessions offered by The Fascia Clinic, specialising in Integrated Myofascial Release Therapy (iMFT), Women's Pelvic Health, and Self-Myofascial Release Therapy (SMFR) with Alexandra Shave ("we," "us," "our"). By registering for or attending treatments, workshops, classes, or sessions ("sessions"), you agree to comply with these Terms and Conditions. If you do not agree with any part of these terms, please refrain from participating in our programs.

Session Participation Requirements:

• Eligibility: Unless otherwise specified, participation in sessions is open to individuals 18 and older. Minors must be accompanied by a parent or legal guardian.

• Health Considerations: You are responsible for assessing your physical condition and suitability for participating in treatments and Self-Myofascial Release Therapy workshops, classes, and sessions. We recommend consulting with a healthcare professional before participating if you have any pre-existing medical conditions or concerns.

Treatment policy:

With this, the Client ("Client") freely consents to receive treatment from:

Alexandra van Dijk - Shave, Licensed Integrated Advanced Myofascial Release Therapist ("Therapist")

Any information given before or exchanged during Treatment is confidential and is only used to provide the best care.

During the Treatment, mobile phones are turned off or kept on silent for optimal focus and relaxation. 

The purpose of the treatment I receive is to reduce the effects of restrictions in the fascial network and to calm down the nervous system. If I experience any pain or discomfort during the session, I will immediately inform the Therapist so she can make adjustments to meet my comfort level. I further understand that Integrated Myofascial Release Therapy should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician or other qualified medical specialists for any mental or physical ailment I am aware of. I understand that the Therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness and that nothing said during the session should be construed as such.

Because Treatment should not be performed under certain medical conditions, I affirm that I have stated all my medical conditions and answered all questions honestly. I agreed to keep the Therapist updated on any changes in my medical profile and understand that there shall be no liability on Cheltenham Holistic Health Centre or on the Therapist's part should I fail to do so. I also understand that any illicit or sexually suggestive remarks or advances made by me will result in the immediate termination of the session. I will be liable for payment of the scheduled appointment. 

CLIENT AGREEMENT AND GENERAL LIABILITY RELEASE FORM 

1. My participation in the Treatment is voluntary.

2. By booking and attending a Myofascial Release Therapy session, you are consenting to receive Myofascial Release Therapy for yourself or any child in your care, and you agree to subsequent attendance for session(s) after this initial consent has been given.

3. I have completed this Intake Form to the best of my knowledge.

4. I understand that Treatment may support my health and is not a substitute for medical care.

5. I understand that the Therapist does not diagnose illnesses or injuries or prescribe medications.

6. Females only: I have informed the Therapist that I am or may be pregnant. I take full responsibility for receiving Treatment during any stage of my pregnancy. I release the Therapist from all liability concerning myself, the pregnancy, and the unborn baby.

7. I have clearance from my medical doctor or (specialist) consultant to receive this Treatment.

8. I understand the risks associated with this treatment include, but are not limited to: a healing crisis, therapeutic pain, short-term muscle/fascial soreness, exacerbation of undiscovered injury, and feeling emotional or tired.

9. I therefore release the Therapist from all liability concerning injuries, emotional and physical effects which may occur during or after the Treatment.

10. I have been given a chance to ask questions about the Treatment, and my questions have been answered.

CLIENT CANCELLATION POLICY:

Good planning is essential for running a busy practice and helping as many people as possible. While there's a waiting list, people often have hectic schedules, making it challenging to fill in cancelled appointments. If you know you won't be able to attend, please give as much notice as possible—two weeks is ideal.

If I cancel or reschedule an appointment on a weekday (Monday to Friday) with less than 48 hours' notice, I agree to pay 50% of the treatment cost.

If I cancel or reschedule a Monday appointment with less than 72 hours' notice, I agree to pay 50% of the treatment cost.

If I cancel within 24 hours or miss the appointment, I will be responsible for paying 100% of the treatment fee.

In case of late arrival, the treatment will end at the agreed time and will be paid in full.

Also, I acknowledge that I must make full payment for any appointment I attend, regardless of the outcome.

Cancellations are best made online (on the Cheltenham Holistic Health Centre website by logging into your account or emailing alex@fasciaclinic.uk.

First appointment policy:

A 100% treatment fee is required as prepayment to confirm your FIRST treatment session. Unfortunately, your first appointment can not be guaranteed without this prepayment. Any follow-up session can be prepaid or paid on the day at the centre.

Limitation of Liability:

While we strive to provide accurate and effective Integrated Myofascial Release Therapy and Self-Myofascial Release Therapy instruction, we cannot guarantee specific results or outcomes. We shall not be liable for any direct, indirect, incidental, or consequential damages arising from your participation in the sessions.

Governing Law:

These Terms and Conditions shall be governed by and construed by the laws of the United Kingdom. Any disputes arising from these terms shall be subject to the exclusive jurisdiction of the courts in the United Kingdom.

Changes to Terms and Conditions:

We reserve the right to update or modify these Terms and Conditions without prior notice. Any changes will be effective immediately upon posting on our website. Continued participation in our sessions constitutes acceptance of the revised terms.
